3-Methylfentanyl (3-MF, mefentanyl) is an opioid analgesic. 3-Methylfentanyl was first discovered in 1974 and subsequently appeared on the street as an alternative to the clandestinely produced fentanyl analog α-methylfentanyl. However, it quickly became apparent that 3-methylfentanyl was much more potent than α-methylfentanyl, and correspondingly even more dangerous. 3-Methylfentanyl has IUPAC name (RS)-N-(3-methyl-1-phenethyl-4-piperidyl)-N-phenyl-propanamide

3-methylfentanyl is the monocarboxylic acid amide resulting from the formal condensation of the arylamino group of 3-methyl-N-phenyl-1-(2-phenylmethyl)piperidine-4-amine with propanoic acid. It has a role as an opioid analgesic, a mu-opioid receptor agonist, and a sedative. It is a member of piperidines and a monocarboxylic acid amide.

In its effects, it is analogous to Fentanyl but is far more potent. 3-Methylfentanyl is 10–15 times more potent than Fentanyl, and several thousand times stronger than Morphine. Side effects of 3-Methylfentanyl are similar to those of fentanyl itself.
